To Schedule An Appointment

In general, patients will need to be referred to us by the physician currently managing your pain. Information from your referring physician is important for understanding the nature of your pain and what procedures or medications have tried. A physician referral form is available here.

Some patients may be ineligible for treatment due to existing medical problems such as significant heart, lung, kidney or liver disease, or diabetes requiring insulin for management. Dependency on even large amounts of prescription pain medication is NOT a reason to exclude you from treatment.

Please call our office during office hours to schedule an appointment. We can generally schedule your first appointment within a week of your call.

Basic blood work also will be required within 60 days of beginning treatment. Patients over 50 will need to provide a copy of their latest EKG.
